ATHLETIC PACKET AND SCHEDULES
Page 1 Medical release form, to be filled out and signed by the student’s doctor stating student is in good health and is able to compete in middle school athletics.
Page 2 Authorization to drive; this form permits you to drive students to away games. This must be filled out even if you plan to drive your child only. See Coach Curtis for more details or questions.
Page 3 Statement of understanding; this is a general form to let the parents and students know what athletics at C2E are all about. Please read and sign the form at the bottom.
Page 4 Practice and game schedule.
All forms and fees are due no later than 11/7/08 at 4:00pm.
Practice starts on November 10th for girls, and November 11th for boys.
You must have all forms and fees in before you can practice.
